I currently have: Windows 10 Enterprise LTSC 1809 installed. Is it possible to upgrade to 1903 or higher without having to download and install a whole new ISO?
When I try Windows Update it says my windows is up to date?

In my opinion, it's better to do a clean install as you will have better results vs running it as an upgrade. You won't get a version upgrade to a newer version via Windows Update on 1809 LTSC, even though it's up to date.
You will have to grab an ISO of a newer version then install it clean.
